Situation: When Veterans transition into civilian life there is an expectation that they will receive the same level of care as they have received throughout their career.
Mission: By empowering Veterans, the Hybrid Care Clinic’s Mission is to provide sustaining high-quality health care to Veterans, their families and families of serving members of the Canadian Armed Forces.
Vision: Is to put in place a primary care delivery model for our target population that:
- optimizes the health outcomes of Veterans, their families and the families of serving members of the Canadian Armed Forces;
- ensures best practices are adhered to;
- has a continuous quality improvement process; and
- be established as the employer of choice for transitioning and already retired health care professionals.
Strategic Objectives:
- Improve the Health and Wellness of the Veterans, their families, and serving members families;
- Optimize the delivery of high-quality evidence-based health care;
- Optimize the use of resources and alignment of our partners; and
- Optimize research, technology, and innovative medicine.
Health care is, at its very essence, serving people. When a member transitions into civilian life there is an expectation that they will receive the same level of care as they have received throughout their career. Unfortunately, in today’s reality access to healthcare can be challenging and the lack of support creates a level of mistrust for Veterans. However, the goal of all aspects of the Hybrid Care Clinic is a process of transitional renewal. A process which has been task tailored – to serve Veterans and their families better.
Health care in Canada is changing, and the Hybrid Care Clinic model will allow our team to be at the vanguard of that change. We are incredibly proud of the contribution of all members of the team involved in the development of this Hybrid Care Clinic Model. They have consistently acted with flexibility, openness, and with the best interests of Veterans at heart. The result is a model which will achieve the dual purposes of allowing our health care personnel to achieve professional fulfillment, and of providing the best possible primary care services to Veterans and their families.
In support of Veterans and their families, Hybrid Care Clinic is mandated to meet the individual health care needs of Veterans in an appropriate, responsive, and timely manner. In essence, Hybrid Care Clinic mission is based on the following objectives:
- Sustaining high-quality health care in the ever-changing environment, and
- Enhancing support to Veteran’s families and the families of serving member
- Offering Primary Care Services to Serving member’s families.
The guiding principles underlying “Veteran-Centered” health care is:
Putting the “Veteran” at the forefront of service design.
A “Veteran-Centered” model ensures that the approach to the organization of activities and expenditures of resources are directed at achieving the most efficient and effective delivery of health care.
A Hybrid Care Clinic Model was trialed in Edmonton AB in the Fall of 2019. Based upon lessons learned from the trial, the plan is to take the clinic model to the next level for the Fall 2020. More details to be promulgated shortly.
